Yes, they use a simple integrated ladder. In this case an old 747-329SF when loading ladder is removed. When finished 1 position is left open to put the ladder, so cabin crew can check during the flight the main cargo deck.
Interesting shot! Is it common to just have a simple ladder for entering the cockpit of 747 freighters?
