Notes:

On 26Jun2006, the RAN released the Board of Inquiry Report on the crash of 817 Sqn Sea King Mk50a N16-100 (Cn WA788) Callsign 'Shark 02' at Nias island Indonesia on 02April 2005. The crash tragically claimed the lives of nine ADF personnel (two passengers survived the crash). Sincere condolences are extended to the families of the personnel involved. This close-cropped photo shows the front end of the same helicopter at RAAF Fairbairn ACT (YSCB) after flying from NAS Nowra (YSNW) on 14Jul1977.
